**Art Sales**:
- Are you planning to sell your artwork at the event? If yes, please indicate the **Price** of each piece.
- Understand that Kiwanis will retain a **25% commission** on sales.
- Decide on your preferred **Payment Method** for the proceeds from your sales.
- **Final Answers**: Ensure that you review your responses carefully. Once you submit the forms, your answers are considered final.

Use one of the forms below to submit your ZozoFest artwork:
**SINGLE ART SUBMISSION FORM**:
Use this form if:
– You are submitting a **Single Piece** of artwork.
– You are submitting **Multiple Pieces of Artwork** that are each different.
Note: YOU must FILL OUT this form for every piece that is different.
**MULTIPLE IDENTICAL ART SUBMISSION FORM*:
This form is required if:
– You have **Identical Items** for sale, such as earrings or ornaments.
– Small variations, like hair color, on otherwise identical pieces do not require separate forms.
